Intensive Case Management
Intensive Case Management is for individuals who have a sanction against them by the Department of Social Services. The Intensive Case Manager must communicate and work with these individuals to help them comply with requirements of Social Services.
The Case Manager does the following:
- Gathers clients' history, and identifies missing pieces to become fully compliant with the DSS case.
- Plans a course of action with the client to prevent or lift a sanction from Social Services
- Provides information, support, and referrals to meet needs such as GED classes, VESID, parenting, counseling, work sites transportation, etc.
- Educates clients in public assistance requirements and provide support for follow-through.
- Advocates and coordinates services with other professionals, and provide transportation if necessary.
*Referrals for the program come from Livingston County Department of Social Services, offering additional supports for clients with specific circumstances.
